Please fill out this form to provide us the info we need to set you up as a resource provider for the Make the Move Conference on November 20, 2025. It is important that you carefully follow the directions listed with each question. Applications are due November 10, 2025.

Your Booth name (business name listed on the headline of your booth).

Tagline, think short and to the point.

Upload a .jpg of your logo. Please save it as 500x500 px.

Upload a pattern or solid color image (no text) that will serve as the background of your booth. This should match your branding and will be the first part of your booth attendees see. This must be a .jpg and less than 2 MB in size.

One - two sentence description of who you are and what you offer.

Video URL to play in your booth. Your video should be 30-90 seconds introducing yourself and what you offer. Encourage people to click the button on the right to connect with you. Video must be uploaded to supported video host including YouTube, Vimeo or Wistia.

Website link - this is the URL you want people directed to from your booth. This should be the landing page for your free offer, not just your website URL.

Twitter link. Please insert actual link, not just your handle (optional).
